Today I want to explain something I see all the time: Hoof pain can be a major contributing factor to insulin resistance and insulin dysregulation.

This is one of the biggest reasons I always look at the feet. Hoof pain is not just a local issue. It is a full body stressor that changes your horse’s internal chemistry, posture, movement, and balance in ways most owners never realize.

When a horse has shoes year round, low heels and long toes, poor angles, contracted feet, chronic inflammation, weak frogs, trouble standing for the farrier, constant weight shifting, tight shoulders and scapulas, or a dropped posture behind the scapula, their feet often hurt. And pain creates stress. The body reacts to hoof pain exactly the same way it reacts to fear, pressure, or injury. Stress is stress, and the physiology is the same.

Here is the chain. Hoof pain creates chronic stress. Chronic stress raises cortisol. Cortisol is the horse’s main stress hormone. Cortisol tells the body to release glucose for quick survival energy. When stress is temporary, the system resets. When stress becomes constant, cortisol stays high. When cortisol stays high, glucose stays high. And when glucose stays high, insulin has to rise to manage it. Over time, the cells stop responding properly to insulin. That is insulin resistance created entirely by pain.

Again: Hoof pain leads to stress. Stress leads to elevated cortisol. Elevated cortisol leads to elevated glucose. Elevated glucose forces elevated insulin. Elevated insulin leads to inflammation. Inflammation leads to more pain. More pain leads to more stress. The cycle feeds itself.

This is why hoof pain is not just a soundness issue. It is a metabolic issue. It is a digestive issue. It is a well being issue. A horse living with chronic hoof discomfort is living in a state of chronic stress. Chronic stress changes behaviour, posture, digestion, and emotional wellbeing. It absolutely changes the horse’s internal chemistry. Elevated insulin does not sit quietly in the bloodstream. It tightens blood flow in the feet, increases inflammation, activates pain pathways, and makes hoof pain worse. And worse hoof pain creates more stress.

This is why some horses slowly decline even when nothing obvious has changed in their environment. Something has changed. Their internal systems have been struggling under the weight of chronic stress created by their feet. Because hoof pain often becomes normalized or dismissed as “just how the horse moves,” the stress it creates goes unnoticed. But the body never stops responding to it.

Hoof pain, stress, cortisol, glucose, insulin, inflammation, more pain, more stress. It is one continuous loop. And it is one of the most overlooked pathways into insulin resistance.

Addressing hoof comfort is not just about soundness. It is about metabolic stability. It is about nervous system regulation. It is about restoring the horse’s ability to move freely without triggering a stress response every time they take a step. When we support the feet, we support the nervous system. When the nervous system calms, cortisol drops. When cortisol drops, glucose stabilizes. When glucose stabilizes, insulin stabilizes. And when insulin stabilizes, the entire body begins to heal.

Holistic care means looking at the whole picture. Diet matters. Minerals matter. Gut health matters. Environment matters. But hoof comfort is one of the most foundational pieces of metabolic health. A horse cannot regulate stress chemistry if every step hurts.
